We use these symptoms as markers to indicate your progress to optimal health and wellness.
At the Human Form, we do NOT treat or diagnose any condition or disease. We have a Medical Advisory Board that handles these services, when appropriate.
Other programs don’t have the ability to measure and assess the vast systems of the body, and how they play a role in your health and reaching your goals.
We monitor symptoms and use numerous (and painless) Assessment Protocols (30-90 depending on your needs) and subsequent data to check your progress and guarantee your overall success.
How It Works
Step One: The Assessment

An Interview to clarify your goals and gain a clear understanding of the process you are about to undertake.
The Assessment is a comprehensive evaluation of your particular strengths and weaknesses in many areas, including flexibility, range of motion, core strength, breathing, movement patterns and much, much more.
The Assessment takes between 90 minutes to 2 hours.
Step Two: Review of Findings
Three to five days after your assessment, we will meet with you and go over all of the details regarding your current condition. We will recommend a plan of action based upon our findings and your health and fitness goals and aspirations.
You are under no obligation to undertake any of our recommendations. You are in control of how much, or little, you would like to take on at any given time.
Our recommendations may include:
- Stretching Protocols
- Exercise Protocols
- Nutritional Protocols
- Lifestyle Re-Design
- Neuromuscular Therapy
- Hormonal Testing (with your MD)
Step Three: Program Design
After choosing the speed at which you want to proceed, we will design programs that detail out the step-by-step plan for your guaranteed success. This will include any of the areas mentioned above.

Step Four: Coaching
Based upon the protocols chosen, we offer a variety of options for private one-on-one coaching.
These will be instructional sessions and/or complete workouts. You can see us for workouts on a regular weekly basis, or learn your routines over a few sessions with us, and then workout on your own.
Other types of sessions can include Lifestyle Coaching, Metabolic Typing, or Sports Psychology.
Step Five (ongoing): Re-Assessment and Program Progression
We will complete a shorter re-assessment approximately every two months, in order to ensure that objective progress is being made at a pace that pleases you. Your program will be re-designed at this point, in order to continue your progress and reach a state of optimal health and fitness.
